The CDC card I got from the hospital who administered both my Pfizer doses is NOT numbered, QR coded, or anything like that.
Vaccine type, lot number, expiration date and dose administrator name are listed for each dose.

There are state databases that probably feed to a national database.
I checked my state database recently and it showed my Covid, shingles, and flu vaccinations. It didn't show any vaccinations that I've gotten on a nearby military base, only those I've gotten at Walmart, CVS, Walgreens.

That’s all I got. Safeway recently sent me an email asking if I wanted it converted to a QR code. After that if the QR code is ever scanned you get a text asking if you want to release the information.
