I did my best to list contacts and phone numbers for supervisors (usually a Sq/CC) and HARM office if I could find it. The further back I went, obviously it wasn't current, so I put the info that I last knew was correct. I had good contacts and phone numbers for my last two PCS's on active duty, but beyond that I put what I had to the best of my knowledge.
I've received no requests for additional info from the background check folks yet. Starting class in a month.

Southwest still asks mil folks to fill out the PRIA 10-year employment paperwork. I know they used the info to call my current flying supervisor (ANG) as a reference.

How do they ask that you turn Stack 1 & 2 in? Right now I have two generic file folders with the Stack 1 and Stack 2 checklist on the front of each and I'm putting the required documents inside. Will they just take the documents out of the folders when its time to turn them in? Or do some people actually have them professionally bound? I'm assuming they want the papers free and not stapled or bound...

Two stacks, loose. You put them on the table and one of the PD folks, Ronna in our case, picks them up.
Doesn't matter if you have them organized alphabetically, tabbed, and placed in leather folders. They want them on the table in front of you in the two stacks. I had them in the order listed on the sheet, but I don't even know that it makes a difference.
Also....get your form notarized that needs it unless you like the extra attention. You can get it notarized there, but it is extra work for them.

Spark is right on for sure.
Use all your resources, make a conservative decision, and stick to it...the will likely try to get you to change your decision.
"Thanks for your input Bob, but the weather is below mins at that airfields. We are heading to xxx" is a good response.
Fastest 7 minutes in my aviation career.
The interview prep is very helpful.
They help you think of the tell me about a time stories...but try and not sound rehearsed.
It is good to practice the LOI several times, and debrief yourself.
This is what WE did well together...This is what I could have done better as the captain.
