Also worth noting that design margins in dynamic structures (which includes most things) exist to account for a variety of variables including mfg imperfections, minor wear and tear, but also fatigue cycles. Carbon fiber airplanes are disposable after X flight cycles, then you throw them away.

Rush might have intentionally pushed the hull or other components beyond the rated limit and close to the structural limit... and then crossed the finish line via fatigue from previous dive cycles.

One of the more fascinating aspects of this whole saga is that Rush had actually succeeding in taking the Titan sub down to Titanic 13 previous times. From Wikipedia "In total, OceanGate undertook six dives to Titanicin 2021 and seven in 2022." It sounds like he had also done dives on other wrecks, and additional "test" dives to various depths.

My understanding is that many previous dives aborted due to technical issues without reaching the bottom. Not sure if those numbers include aborts.

Submarine hulls are inherently life limited to a certain number of pressure cycles. That's fairly predictable with metal hulls... steel has the longest life, while titanium will tolerate fewer cycles but is stronger for the weight.

Carbon fiber is problematic, harder to predict and monitor performance.
