In this day of perpetual lawsuits, most "big corporation" type companies, at least outside of the aviation world, vitually refuse to say anything about a former employee other than he worked there during a specific period of time. I find it odd that any businessman would want to take on the possiblity of a lawsuit for something as subjective as a "bad attitude". Then again I met plenty of lousy businessmen in the aviation world.

They cited a bad attitude, that was it. I passed my students off because we were slow. I was not flying at all because there were really no students coming in. I was frustrated about the lack of work and was not going to sit in the office by the phone waiting for new students with out pay. I took a 135 job and that was 8 months ago.
Perception is more important than reality.

The packet at Eagle has a "will not hold liable" type thing to sign off. The PRIA information they send to previous flying jobs includes a checklist with questions like: Did this person follow procedures, did this person show up to work on time, did he work well with others....etc.
I was rejected by the review board at Eagle after a successful interview and I suspect this had something to do with it.
Good luck, hope you fare better than I did.

My last job was a scheduled 135 gig that I only did for about 4 months before getting hired at my current job. I wasn't under a training contract, I got along with everyone, I gave them two weeks notice, and the chief pilot even wished me the best of luck on my last day when I turned in my badge. However, when the PRIA stuff came back from them there was an HR paper in there and they had checked a box saying I wasn't eligible for rehire with a comment underneath saying something about how no one who resigns voluntarily is eligible for rehire.
Kind of a dumb policy for a company but, hey, that's their business. My current employers didn't say anything about it.
