Glad nobody got hurt!....But I have to ask....isn't sombody suposed to be looking out the window and down the runway when the PF pushes the throttles up?

Runway near miss avoided at Allentown airport - Midstate PA Local News, Weather, Sports & Entertainment - PennLive.com

What do your company specs say about starting the roll when there's still another Aircraft on the runway? I know it's happened to me, but I usually wait until they have left it.
