After talking to a few different doctors and resources, there seems to be something called a 'blue ribbon' (FAA Form 8065-2). I've never applied for a medical before, one psychiatrist said that this form typically takes 2-3 months for them to receive, and the FAA rep said it probably is blank anyway. One said trying to call my AME to see if they had a copy of it, but my AME doesn't return calls, emails, texts, messages, etc. and has written me off to find all of this information for myself.

Is the psychiatrist still able to perform an evaluation without this 'Blue ribbon' form if I provide all of the other information requested?

I heard it's been taking 2-3 months for this form to be received by doctors. Is there any way he can do the eval without the form or do I need to just wait this one out?

1. Click on link.
2. Wait for PDF form to load.
3. Select “print” on your computer.
4. Remove printed copy from your printer


You now have the form.

IF YOU WANT TO CONTINUE ON MEDICATION

This is the procedure you need to follow:

This is a list of neuropsychologists they recommend who are familiar with the testing they require. You don’t HAVE to use one of them, but it might expedite the process:

IF YOU HAVE BEEN OFF THE MEDICATION FOR 60 DAYS AND NEITHER NEED NOR DESIRE TO GO BACK ON:

Basically, this requires you to get an assessment by your treating physician that your depressive symptoms have resolved, that you are no longer on antidepressants and have not been on them for (insert time 60 days or greater) and that he/she is not recommending that you have additional treatment.

It would be helpful if he/she could include that you had been given one or more of these standard screening tests and tested normal:

The Patient Health Questionnaire-9 (PHQ-9) -- a 9-item self-administered diagnostic screening and severity tool based on current diagnostic criteria for major depression
Beck Depression Inventory (BDI), -- a 21-question multiple-choice self-report that measures the severity of depression symptoms and feelings
Zung Self-Rating Depression Scale -- a short survey that measures the level of depression, ranging from normal to severely depressed
Center for Epidemiologic Studies-Depression Scale (CES-D) -- an instrument that allows patients to evaluate their feelings, behavior, and outlook from the previous week
Hamilton Rating Scale for Depression (HRSD), also known as the Hamilton Depression Rating Scale (HDRS) or abbreviated to HAM-D -- a multiple choice questionnaire that doctors may use to rate the severity of a patient's depression.

If your prescribing physician was NOT a psychiatrist, just go to a psychiatrist who is knowledgeable about these screening tests for depression, explain the situation to him/her, provide copies of whatever records are available from back when you were being treated for depression and get them to do the screening test and write the letter.

This isn’t rocket surgery, you are making this much too hard, and you are starting to depress me. Open these links and study them, particularly the bottom one. Make a copy of it to take to see the shrink, preferably in color. You are shooting for the result in the upper right yellow box, a REGULAR ISSUANCE, I would presume?

I think you misinterpreted that part, as I was asking about the blue ribbon file (psych explained it to me so I have that answer now), but thank you for the other information!
