I would just call them back up and tell them about the issue and explain you totally forgot about it, but want to disclose it now at the beginning of this process as opposed to them finding out in the Phase II background check. Shouldnt be a big deal, I am guessing they will just send you another form to fill out and all will be good.

Reserve and first year pay
I’m currently in Phase 2 and I hoping for a class date shortly after the first of the year. But who knows?
I’m very excited to join the jetBlue team. However, I am a little worried about never breaking guarantee, not earning much per-diem and being on first year pay.
I have a couple of questions.
How long before health care benefits are provided?
While on reserve if I pickup flying on my day off is it paid out above guarantee?
If so does it pay out at regular hourly rate or greater?
Can I pickup open time out of other bases?
Is the pay different if I pickup something from open time compared to picking up a turn from another pilot?
Is there a minimum credit for a call out while on reserve?
What is the best way to maximize the pay check while on reserve?
What is the best way to maximize the pay check when holding a line?
THE BIG QUESTION..... What equipment and base should I bid for in class if the most important thing is to get off reserve?
During the last 7 years at my current airline I have never been the person to pickup any extra flying. That being said, I am experiencing some anxiety over first year pay. I can afford the pay cut but it’s going to suck. I have become a custom to the extra money I have been earning. Any info you guys can provide is greatly appreciated.

1st of the month after 30 days.

Not right now. it was supposed to be but it is currently on hold.
If so does it pay out at regular hourly rate or greater?
See Previous. If you happen to go over 78 hours on reserve anything you fly or pick up at that point would be paid at 1.5
Can I pickup open time out of other bases?
No. You can pick up extra reserve out of base is offered. Paid above guarantee.
Is the pay different if I pickup something from open time compared to picking up a turn from another pilot?
No. Everything above 78 hours is paid at 1.5.
Is there a minimum credit for a call out while on reserve?
5 hours min duty period. Goes towards guarantee.
What is the best way to maximize the pay check while on reserve?
Pick up extra reserve if offered. See above.
What is the best way to maximize the pay check when holding a line?
Pick up above 78 hours.
THE BIG QUESTION..... What equipment and base should I bid for in class if the most important thing is to get off reserve?
It has been E190. I would say for anyone coming in from now on it will be airbus. We are taking more airbus(about 8-10 a year) and only a couple 190's(4ish) each year from now on.

Hang in there, should the hiring stay as it did in 11" I was on reserve two months, in BOS. Reserve life was not bad, actually was 100% better than 10 year seniority at my last airline, not exagerating. Pay is not too bad on reserve, and once you hold a line, be creative on FLICA, its a great tool! Good luck.

Congrats! I interviewed in mid October and am currently in Phase 2 on the process. It was exactly one week after the interview that I was notified by email. Good Luck.
