HVN @ night in winter with contaminated runway not reported because tower is closed?
"At the time of the accident, the first officer had only 18 hours in the Boeing 737 and most of his
previous experience was operating light aircraft. Thus, his lack of experience flying jet aircraft
likely played a role in his inadequate monitoring of the approach (his lack of experience was
also exemplified by his failure to note, as part of his monitoring duties, that the speedbrake
handle had not been armed after calling the item as part of the Landing checklist)."

I thought it’s interesting from a cultural aspect. Either the Captain was a risk taker and a company man OR was coerced/threatened. Sometimes you just have to say no! Reading about management types and cultural implications, this is why it should be here. There’s another reason as well but I’ll let others bring it up.
It’s an important report and applicable to the aircraft type, flying into a high threat airport, and a somewhat similar type of op. It’s a good topic of discussion particularly hiring DECs and low time FOs and the issues that brings.

We have enough to worry about with small airports, we don’t need management being cheap about our paychecks or firing people because they don’t agree with them. The culture here is toxic that was created last year. It’s a regional airline at best where excuses and threats are made on a regular basis.
I know management and some management look over these boards trying to figure who is who. Be careful on what you post. We were even told in a meeting that discipline action can be taken for posting information. That’s right, they even threaten people with posting on a public message board.
It’s no way to run a safe airline.

This was a cluster Fk that should have never had happen. This guy is a Check Airman that should never have been a Check Airman. This is a training failure in a colossal way. Now two careers are ruined by an airline.

Protect your career, stay far away from this place until they can get their act together.
