I have put in parenthesis the city where I experienced this. Where I did not have a city, I just couldn't remember or it happened at every hotel!
Bed bugs (Los Angeles)
Strange curly pubic hair, not mine or hubby's (Idaho Falls)
Air conditioner that gives off heat (MIA, MNL, HKG)
Used feminine products in trash can of a "cleaned" room (Austin)
Yellow stains, albeit faint ones, on pillowcase
TV remote buttons stuck because of strange dried substance
Going to "my" room only to find two people already sleeping there (Orcas Island)
Single ply toilet paper
Room clock radio shows a time that is 5 hours off the correct one (San Francisco, Tenderloin district)

If the clerk is being honest with the order, then why does it matter? I know that if I call a hotel and a clerk asks me to hold (especially when they say that there are people in line), I understand and don't mind one bit. A queue is a queue, whether it's on the phone or in person. It's pretty cut and dried.

I think one of the most inexcusable gripes is when I have a late check-in and then a late check-out due to my airline schedule yet they code my key to expire at the "normal" checkout time, forcing me to make an extra trek to the front desk to get access to my room!
Totally avoidable if they'd put a little attention to detail!
If we were that careless about our job, we'd be all over the front page of the news!

No outlet next to the bed (#1 by a longggggggggg shot)
People doing that stupid deadbolt trick at 4am
house keeping that cant see the do not disturb sign.
AC that doesn't have a fan mode (i now have an app) or sounds like the space shuttle lifting off
Last but not least is when they toilet paper holder keeps the spare roll behind the "active" roll but yet its so tight that you have to manually unroll it off the holder or you just get tiny pieces of confetti. I can provide pictures. place im at now has this exact issue.

Commercial-grade door closers built for windy conditions installed in a no-wind setting. I really enjoy fighting the door to get my bags though it, don't you?
"Pillows" and "mattresses" that were designed as industrial packing material, not as bedding.
Non-smoking rooms that have obviously been smoked in.
Desk staff that get annoyed because, SHOCKER!, a guest needs some customer service.
"Broadband internet" that doesn't resemble anything close to it.
The stupid 1 cup coffee makers that are actually designed to kill you, not brew a cup of drinkable coffee. And while I'm at it, only one package of coffee, and it's decaf.

Noise ... especially when I have to get to bed early or sleep in due to my report time. It's bad enough from the other guests but loud banter from the maids (or them knocking on my door) is something they should be aware of as part of good customer service training.
By the way, I started using a 'white noise' (background noise) machine app on my phone on my layovers and it seems to help cut down with being awakened by noises. There are several available but I use "White Noise Free" (TMSOFT) and I like it. It's available for both iPhone and Android.
