All these tips so far are spot on. I've found that one item that DOESN'T travel well are bananas. After three years of trying to tote them around, I've finally given up. After even one minor bruise, they start to get squishy and leak all over your bag, leaving a disgusting, sticky, smelly goo over everything.
Also, don't forget to clean everything out as soon as you get home. Nothing sucks worse than having ten minutes before you need to run out the door for a 4-day trip and opening your lunchbox to find a fledgling compost pile...

Tortillias
I think only one person mentioned flatbread or pita bread. I personally just use regular tortillias with sandwich meat and cheese. Lettuce is a bit tricky so that usually doesn't make it. Then a small travel bottle of mayo or ranch and your done.
I usually have enough for 1 per day and then I also pack a hot meal such as spaghetti, hamburger helper, grilled chicken w/ some type of vegetable. I have also done stir fry, pork chops and meatloaf.
All in separate tuperware that I saved from all the deli meats I purchased. I can easily get 4 days worth of meats, hot meals as well as random snacks like granola bars and oatmeal in a standard size crew lunch bag.
