You guys have been so much help. I guess my next step might be to go ahead and get a background check run on myself, and get in touch with an AME. Good thing is I still have time I'm still in ground school so I won't even sit in a plane for another 4-6 months. One more thing this is probably a long shot, but does anyone happen to have a scan of the medical evaluation form or maybe one in a PDF. file I can look at?

Don't mention the asthma. The FAA will never know about it unless you tell them. You never had it so there is no reason to open a can of worms. Don't worry about it anymore.
If you have to mention the drinking I don't think it will be a big deal. Make sure you actually do have to check that box - don't just assume you do. That should not prevent you from getting a first class, and it should not have a negative affect on your career (as long as you don't repeat the same mistake again).
