I don't have a strong opinion on it. I'm just parroting the folks who get to make up the rules and who bring the certificate actions.

I almost went for a MLS degree (bet you they don't get furloughed!) Took an undergraduate class and the instructor said, "You don't need to know everything, you just need to know where to find it."
As NGDI says, this just came out last month. But I'd think the author would at least say he'd clean that up on the next revision. Fast forward to the administrative law court room. You're sitting there in your suit clutching Mr. Lengel's book. The FAA lawyer starts with the fact that you entered Class B airspace without a clearance. "But your Honor, Mr. Lengel says it's OK if I was on an assigned heading." "Your Honor, the FAA, who writes the rules and enforces them, has made its opinion available to pilots since January of 2010."
The author says he's entered CLTs Class B on a heading for years. CLT approach may not have known what Washington thinks. I've had a bunch of F/Os check if we're cleared across a runway (not the one we've been cleared to.) Ten seconds versus the above court room scene.

The thing I'm getting hung up on are the words he used that "if you just fly into class B airspace" be it heading or altitude, and you didn't get a clearance, it's ok because you're not supposed to deviate from ATC instructions.
Well then, it's the pilot's job to request amended instructions if the one he is getting will make him break a FAR isn't it?
If ATC vectors you into a hot restricted area VFR, or gives you a climb that takes you into a cloud layer, and you do it, are you gonna sit there and tell the FAA that "but ATC made me do it !!"?
That's where I feel the book-guy's argument falls short.
No apples or oranges:
- If you do not hear the words "cleared" somewhere, you are not operating with a clearance, just ATC instructions.
- If you enter a class B airspace, without a clearance, you are at fault for not requesting it, or deviating to avoid breaking a FAR
And lastly, in regards to someone not affiliated with the TRACON not having the jurisdiction to clear you through a class B airspace VFR I think this is wrong as well.
They can't do it on a whim, but I believe they can talk to a class B controller and get that clearance for you.
I have at least taken of from a Class D airport below a class B airspace and gotten my clearance (VFR) from the Class D controller
