1. 100% 401k matching. You put in 8k and they'll match with 8k.
2. Employee profit sharing. The "Charter company" is owned wholly by a private oil company and the employees receive anywhere from $10-$20k bonus each year.
3. All overnights, or layovers of 2+hrs, and you get a rental car.
4. No per diem but you submit all bills and they cover them.
5. They pay 100% for all medical and dental insurance. Vision is free eye exam with $200 towards contacts or glasses per year.
What else do you guys get with your departments? What are you schedules like? This place is 4 weeks on with 1 week off. On average they fly 15 to 16 days during your 4 weeks. Laid back environment. You can ask for time off when you need it. If you're out having a beer they just call the next guy type place. On trips with overnights you can bring your golf clubs and they'll pay for your round if you go(was told if you hand in a bill for over $150 then you might have to answer a few questions). They also only hire from within. If a new jet is brought in they don't hire off the street for it so you can put your name in the hat.
What other questions should I ask?

The benefits sound normal, although I have always had to pay into my medical/dental etc. So good for you!
Expenses are usually not per-diem, rental cars are the norm in all but the lowest places...no shuttles to the Holiday Inn thanks, and no hanging around pilot lounges with the fractional guys snoring all day...common sense and consistency on the expenses seems to work just fine.
Bonus is bonus...ask them how many times in the last 25yrs it has been paid. Don't ever consider it salary because they can ditch it anytime. I hate the bonus system unless the salary is also high, then its a true "bonus"
All the other stuff is questionable...."Laid back??"...that always scared me. Whats laid back mean? Don't believe for a second that nobody cares if you are into the beers when they call you for a trip either, because they sure as hell do. I'd rather just know the rules and follow them.
The little perks like golf etc sound great, but I'd rather you just pay me very well and not analyze my spending. I don't want anyone picking up my golf or worrying about crossing the imaginary limit line. Work is work and I want my personal life to remain mine...but that's just me. As a new guy, I'd stay off the radar in these areas.
EVERY corporate department (2 pilot to 60 pilot) has its politics, and its stuff like this that very often does NOT work out in your favor.

Absolutely, if you are moving you should ask.
I have seen it really not be part of the package, but once someone inquired a token amount (say 10K) was offered...or reimbursement for moving trucks, hotels, meals etc.
I think it varies on the level of job...sometimes its nothing for entry-level pilots. I have also seen a years housing covered on both ends and all moving expenses (total up to 75K) for Chief Pilot type jobs.
Ask? hell yes ask!...you would be amazed at how many pilots DON'T ask.

I will just add that relocation is usually covered by any decent gig. Some may have limits, but more often than not, you won't come close to those limits.
Some other things I would ask would be the loss of license insurance as well as company-paid life and AD&D insurance.
Definitely ask about bonuses, and no, I also wouldn't consider it as part of the salary.

This depends a little bit on your experience and currency level. If you're bringing a type rating and currency into the deal I would absolutely ask for moving expenses. If they're gonna buy you a type rating out the chute and you're not ready to hit the ground as a line captain you might have a harder time asking for moving/ lease termination expenses. I'm not saying you can't do it, though. Definitely ask. Good luck!!!
