Safety Reports or Confessional Booth??
I can't believe some of the lame cr*p I read in some of these safety reports. Is this a safety report we can all benefit from or someone looking for absolution by going to confession?
Operating FDXxxx, Kxxx-KMEM. Using the newly installed "Electronic
Flight Bag" (EFB) with the LIDO charting system. This was my 5th
opportunity to use the EFB, but the first time in daylight. I was
having some difficulty reading the notes on our company parking
pages due to the small font being washed out in daylight
conditions. I missed reading the note that ATC clearance was
required for pushback. We called PHX Ground for taxi instructions
after pushback, and the controller did not query us for pushing
back without her clearance. During cruise, I was exploring the EFB
and then found the note mentioning ATC clearance was required for
pushback.
In the future, when using the EFB and LIDO charting system, I will
expand the magnification of the applicable page, especially in
daylight, when I see notes in small font.
Response:
ATC had no problems with the crew of this flight. No further action
is necessary. This information was shared (de-identified) so
others can learn from this event.

We do at Brown, and it's extremely successful. When guys "screw up" by accident (I've done it once, I think ) and can SHARE their experience without fear of punishment from the company or FAA, it's a HUGE benefit for the pilot group as a whole.
ASAP programs also help the company and FAA identify trends and change the annual training program syllabus to address those issues which otherwise would never have come to light.
It's the trivializing, belittling attitudes like your post that would keep the modern aviation system locked in a hidden, less-safe operating world.

I just love how the F/O's end up sounding like steely-eyed missile men.
"Lost #1 engine at V1. Tower reported flames coming out of tail. Lost #2 engine on base leg, then the other one. Glided in and ran off runway and thru chicken coop, flipping upside down and breaking all the little bitty bones in my legs. The f/o did an excellent job and was a big help....."
