1. Just need a picture to mail and passport will be done.

Another question -- certified transcripts. I can order them but do I need to send them to myself and then bring them to interviews or do I wait until asked by the company and send directly to them?

I will add one more thing to the list.
A copy of the Driving History Printout for every state that you've had a driver's Lic from in the last 5 years.
Another time consuming thing is filling out application at airline apps website.
Gathering all those addresses for the last 10 years can be a pain some time and Segregating all those flight hours into the catagories how airline apps wants can take a lot of time as well.
You can make a free account on airline apps & fill in your flight times, addresses etc etc and have your application ready to shoot to airlines when you are ready..

When I examined the delta addendum they didn't ask about any of the 3 things I originally asked about. I guess I could've just read the addendum instead of posting here . I needed to get all that crap at some point anyway and another responder brought up a good point about making sure I can even get a class 1. I don't think I've had an EKG since my initial physical before entering pilot training.

After the day one interview process, if all goes well, then they will offer you "conditional employment", pending three things:
1. Their medical evaluation of you
2. Their psych eval of you
3. Their background check, which usually isn't a problem with the military given our security clearances, but your driving record will be checked then. The most important thing is to simply be honest about those things.

Similar to having your driving records and school transcripts, once you get your ATP request your complete FAA records. It's on the FAA website: AC Form 8060-68 (10/09). To make sure there's nothing wrong or missing. (This is not an interview requirement but you want to know what's in there)

Different. The PRIA request to the FAA came back to me (I got a copy) as a 1 page document verifying what certificate I had, medical class, no violations or legal actions. The full airman records has every piece of paperwork you ever sent in to the FAA. All your test scores. I believe (but not sure) it would also have any medical waiver paperwork.

Their concern is a pattern of not following the law. If they see that you are getting speeding tickets or other violations every year it will be a problem. Your 2 tickets in 6 years will not be a problem. Just make sure you do not get another one.
