I would highly suggest revisiting the way you study. Systematically unsatting events as you have shown to do points toward the individual either with basic aptitude, habits, or a combination of both.

I am on the fence as what to say here...and perhaps I shouldnt say anything. Here is what I think... I think anyone who tells you its no big deal.... well... imho, they are wrong. It will cause you to have issues getting hired in the future. Note... I am not saying you cant get hired but the reality is with the number of people available right now you will probably have to go to a company that is less than desirable, and you had better be spot on from day one.
Also, you systematically failed essentially two phase checks and a major check ride in less than a year, as hard as this is to say... perhaps you need to take a look at whether you are ready for this type of flying at this point. I know... some will say its early.. its ONLY two phase checks and his ATP!!!! Listen, I am not saying give up if you are sure you want this, but you need to do a top down overhaul of how you study and how you prepare ....in addition do a top down assesment of your abilities...Better yet have someone you trust not to blow smoke up your butt give you HONEST assessments of your abilities.

Sounds like American Eagle. Many people have been screwed over at that airline with the proby check. Just explain how many hours you have in the plane and that you passed your intial with zero problems. Certain regionals will understand that airlines normally don't wash out people who have flown the line over 1 checkride. Unless they are in bankruptcy and might have to let go of some pilots before furlough. I know a few proby victims at skywest and gojet. Be positive and use this as learning experience.

Def sounds like Eagle. Sadly I think their prep for the type ride is inadequate and double sad that because you happened to have enough hours for an ATP they gave you your type ride as your probie ride. Others will get a regular probie ride and then come back next June for their type & prob get more training than what they are giving now.

On a side note, I thought our training unsats were not reportable and were internal only.

Some folks just aren't meant to be pilots, no matter how great their rapport with peers and supervisors is. We lost 10 out of 30 guys in my UPT class due to training failures. They were all "good guys". They just were in the wrong line of work.
This job is more than not abusing sick time and being a nice guy to work with. You need to bring your A game every day because you really never know when you'll need it. You now have a history of not doing that and you need to honestly figure out if you can in the future, for your sake as well as your passengers.

Keep plugging along youngster you will be fine because time eventually heals all. I busted a couple of checkrides attempting to get the licenses such as commercial and instrument over 30 years ago, its nothing but a road bump in my rearview mirror of over 15,000 hours. Good luck to you!

I would say a sim "rewind" and repeat of an evolution for practice during training does not get reported. A PC failure does. A failure of an intermediate training session requiring a repeat may or may not.
The OP needs to figure out exactly what gets reported here by the company in question.

First and foremost, you must take "ownership" of your record, which shows some serious deficiencies. Three "busts" on probation is serious and potentially totally disqualifying in the current environment. It was not the company's fault, it was not the check airman's fault, regardless of what prople here might say, the next job interviewer will hang it on you.
Next, review your past year, what went wrong, why you weren't prepared, what lessons you didn't bring to check rides. If you know a senior-ish pilot to have a real heart-to-heart talk with, it would help. Someone you trust to give a honest evaluation and what might put you back on track. And I don't mean an anonymous forum. A real mentor honest enough to say you should be a carpenter or can really put your career as a pilot back on the rails.
Lastly, this isn't thirty years ago; the age of quick news, law suits, PRIA, and Colgan 3407 makes airline employers nervous and with the number of better candidates out there, consider the possibility of not flying for an airline. Hard as it is, you need to rebuild your reputation by flying lots of time somewhere.
