Default How should I answer this on a job app?

I'm filling out my app on airlineapps.com and this is one of the questions.

"Have you ever received a notice of disapproval from an FAA Inspector or FAA designated examiner for failure of an FAA Oral, Practical, or Flight test?"

I failed my one and only checkride on my ATP way back in '95 because of a boneheaded mistake, but passed the next day. I answered yes and in the "explain box", I put "Failed ATP flight test".

Well, at the end of the page, it asks this question:

"How many total Notices of Disapproval/Unsats have you received for failure of an FAA flight test?"

WTF is a Notice of Disapproval/Unsat? Is that the logbook entry that says "Unsatisfactory flight check"?

Yes, Unsat = Unsatisfactory. So if you've failed one checkride, you answer 1.

That's what I figured.

Never heard of a Notice of Disapproval/Unsat.

Notice of disapproval used to be pink.
Notice of disapproval = unsatisfactory = failed checkride

"Notice of Disapproval" is the official name of the form.

Commonly referred to as "Pink Slip" in the aviation community. One and the same.
